When something is unsafe, staff don’t just tick “fail” and move on. They capture the evidence there and then, and create the next step immediately:

A physical hazard (broken flooring, damaged handrail, blocked exit, faulty lighting) becomes a maintenance work order with priority and due date.
A near-miss or safety event becomes an incident report with corrective actions and follow-up.
A safety-relevant system issue (for example, CCTV or access system faults where applicable) can be raised as an IT ticket.
